These forms are to use as samples only. The petitioner must request that application materials be sent from the above address. Failure to use the forms sent by the Legal Division may result in rejection of the application.

An inmate must complete the Application for Clemency, and submit it to the Governor. Completed applications should be sent to:

The application must be accompanied by a separate letter that states the extenuating circumstances which support the basis of the clemency request. Applicants are required to include letters of recommendation in support of the request from sources such as neighbors, employers, co-workers, pastors, church members, elected officials, judges, prosecutors, family members, etc.

Legal counsel for the Governor will review the application and will notify the applicant, and the person submitting the application where indicated, of its receipt and sufficiency.

If the Governor believes an application merits further inquiry, he or she will refer the application to the State Parole Board to conduct an investigation. Whatever the recommendation of the State Parole Board, any decision on an application is exclusively the Governor's.
